Private Care homes are suited to those who are no longer independent enough to live in their own homes or a lodge, but do not require the level of care provided by a Nursing home.

The assessment tools help determine whether care can be intermittent, substantial, full-time or around the clock.

Once you have met with the individual operator and discussed the care plan assess to the client you will be required to sign an admission agreement. There will be a re-assessment within 1-3 months depending upon the operator.

 

.... To better help you understand the levels of care each level will be assessed based on the following:


*ADLs and *IADLs are important measures of functional disability and serve as assessment tools for determining appropriate levels of care, and care planning. Additional criteria to supplement ADLs/IADLs include: *behavioral problems, safety management, fecal (versus urinary) incontinence, medical and rehabilitation needs and social functioning.


1-* ADLs (Activities of daily living, e.g. bathing, dressing, eating).

Physical care needs includes partial assistance, cueing/supervision to total assistance

* IADLs (Instrumental activities of daily living, e.g. transportation, shopping, financial management)



2- * Behavioral management

Client requires assistance, some supervision/total supervision due to cognitive deficits including memory, orientation and comprehension.



3- * Safety management

Support needs to promote safety for the client and others in the home.
Inability to recognize and understand safety measures, dangerous to self and others(e.g.wandering,physical/verbal intimidation)
