The following assessment form is a tool that was devised to help operators clarify the level of care that is required by the interested client or family. We feel this helps shorten the time frame involved in finding the suitable Private care home for the potential client.

The panel within Calgary that set up this assessment tool consisted of CHR, CHR Housing Department, Home Care, Kerby Centre, Accessible Housing, Licensing Department and the New Age Personal Care Association.

This tool was developed using the Edmonton -Capital Health region form but simplifying it for ease of use.
